HammondCare seeks to embed evidence-based best practices in its services and we provide these services through Home Care, Sub-Acute Hospitals and Residential Aged Care.<\/p>\n<h3><strong>What&#8217;s involved in the role?<\/strong><\/h3>\n<p>Home Care Support Workers deliver a range of &#8216;At Home Aged Care services and mostly work independently in client homes, but with direct support from their Care Manager. Duties include:<\/p>\n<ul>\n<li>Assisting with personal care<\/li>\n<li>Medication assistance<\/li>\n<li>Meal preparation<\/li>\n<li>Light housework<\/li>\n<li>Shopping and transport to medical appointments<\/li>\n<li>Social support and community engagement<\/li>\n<\/ul>\n<h3><strong>What do <em>you<\/em> need to apply?<\/strong><\/h3>\n<ul>\n<li>No formal qualification is necessary, however, Certificate III Aged Care is an advantage<\/li>\n<li>Transferrable skills in providing care and compassion to others<\/li>\n<li>Valid Drivers Licence<\/li>\n<li>A reliable car with minimum third-party insurance<\/li>\n<li>Certificate III in Individual Support is desirable (not essential)<\/li>\n<\/ul>\n<h3><strong>Culture<\/strong><\/h3>\n<p>HammondCare At Home has a relationship-based approach to care, and our team members take the time to fully understand the individual needs of every person we care for.
